For anxiety and depression claims, do I upload in service record screenshots of notes of me going to behavioral health for anxiety and depression?

Do I also take a pic of my diagnoses for adjustment disorder in service and upload it as evidence? I don’t know what to do at all, I feel like I just want to put the claim in without doing anything because can’t they see the evidence themselves by looking through my military records and all?
